Job summary

CAMBA is seeking a Case Management Navigator to support the Van Dyke Cornerstone Community Center team in Brooklyn. You will work with a Social Worker to implement ISPs, provide outreach, and connect participants with health, housing, and employment resources.

You will coordinate with Cure Violence partners, identify high-risk participants, and maintain trust to ensure meaningful engagement while supporting families and building a strong community network.

Qualifications

• Bachelor's degree or equivalent experience and one year of applicable experience, or
• Associate's degree with two years of applicable experience, or
• High School Diploma with three years of applicable experience.
• Relevant experience in a mid-sized organization and strong written/verbal communication.

Responsibilities

• Work with the Social Worker on the implementation of ISPs.
• Provide outreach and continuous support to participants.
• Collaborate with Cure Violence providers and neighborhood resources to create a comprehensive support network.
• Identify high-risk participants and conduct initial outreach to engage those not in the program.
• Maintain trust with participants and connect them with Social Worker and Cornerstone activities.
• Coordinate activities of Peer Support Leaders and organize development incentives and events.
