Job summary

CAMBA in Brooklyn seeks a Case Management Navigator to support the Cornerstone program for young adults (ages 18-21). You will work with a Social Worker and a Navigator to connect participants with health, housing, employment, and education resources, advancing Individual Service Plans and keeping thorough progress notes.

This role emphasizes outreach, building trust, coordinating with community partners, and documenting enrollment data while supporting families and ensuring timely reporting;

Qualifications

• Bachelor’s degree (B.A. or B.S.) with 1 year of applicable experience.
• Associate’s degree (A.S.) with 2 years of applicable experience.
• High School Diploma with 3 years of applicable experience.

Responsibilities

• Work with the Social Worker on the ISP implementation.
• Provide outreach and continuous support to participants.
• Collaborate with Cure Violence providers and local resources to support participants.
• Identify high‑risk participants and conduct initial outreach.
• Maintain trust and connect participants with Social Worker and Cornerstone activities.
• Coordinate activities of Peer Support Leaders and organize incentives.
• Assist in enrollment forms, documentation, and data tracking to ensure compliance.
• Monitor progress via phone or face‑to‑face visits and document progress notes.
• Recommend closing of cases when goals are achieved or participation ceases.
• Provide information for weekly/monthly/quarterly/annual reports.
• Schedule client re‑assessments and referrals as needed.
• Escort clients to school as needed and ensure safety.
• Other duties as assigned.
