Job summary

ICL in Brooklyn, NY is seeking personnel to support consumers living in independent apartments in the community. You will assist individuals in developing living, working, and social skills, coordinating medical, psychosocial, educational, financial, and vocational services, and providing crisis intervention as needed.

The role involves travel to consumers’ residences to offer counseling and support while promoting maximum independence, with duties often performed under general supervision.

Qualifications

• Bachelor's or Associate's Degree required.
• Experience providing direct services to individuals with mental disabilities or other relevant human service experience.
• Must be authorized to work in the U.S.

Responsibilities

• Travels to/visits consumers' residences to provide supportive services on a schedule established by the program.
• Maintains up-to-date individual case records and develops measurable service plans.
• Provides 1:1 or group counseling to assist consumers to modify attitudes and behaviors.
• Obtains resource information and disseminates it to consumers clearly.
• Uses cognitive-behavioral techniques to support service plan goals.
• Explains clinical services and ICL programs to consumers and teaches daily living skills.
• Performs crisis assessment and initiates appropriate responses.
• References community resources and accompanies consumers to external appointments as needed.
