Job summary

ICL in Brooklyn, NY, seeks a compassionate practitioner to support individuals with mental disabilities living in community settings.

Under supervision, you will provide crisis intervention, advocacy, service coordination, and outreach, travelling to residences to promote independence and community integration.

Qualifications

• Experience providing direct services to individuals with mental disabilities or related human services experience.
• Ability to travel to and support consumers in community settings.
• Strong documentation and case management skills.

Responsibilities

• Support consumers in independent living by coordinating medical, social, psychosocial, educational, and vocational services.
• Travel to consumers' residences to provide counseling and assist with service plans.
• Maintain accurate consumer treatment plans, case notes, and quarterly reviews.
• Provide 1:1 or group counseling and crisis intervention as needed.
• Refer clients to community resources and accompany visits to medical or social services.
• Ensure safety, complete incident reports, and participate in supervision and training.
