Job summary

Service Coordination, Inc. in Pikesville, MD is hiring Service Coordinators to advocate for underserved individuals and deliver coordinated case management in the community.

You will develop and monitor Person Centered Plans, connect people to resources, and support transitions to community services while following HIPAA and regulatory requirements. Strong communication and teamwork are essential for success.

Qualifications

• Bachelor’s degree in a human services field or equivalent plus relevant experience.
• Experience working with people with developmental disabilities preferred.
• Knowledge of state requirements for service coordination is a plus.

Responsibilities

• Advocate for individuals, provide resources and educate on self-determination.
• Collect information to determine eligibility for services and secure PCPs.
• Plan, maintain and update Person Centered Plans (PCPs) with outcomes.
• Coordinate services, document progress and assist with waivers and approvals.
• Identify and refer resources; arrange transitions to community-based supports.
• Monitor PCPs, ensure compliance and maintain HIPAA records.
• Collaborate with teams to deliver person-centered, outcome-focused services.
