Job summary

Qualifications

• High school diploma or equivalent required.
• Willingness to travel at least 65% of the time.
• Experience using email and Microsoft Office products.
• Ability to communicate effectively in writing.

Responsibilities

• Attend work on a regular and predictable schedule as required.
• Interview applicants and conduct home/site visits to assess needs and provide information and referrals.
• Develop, coordinate, and review service plans with individuals and families and authorize services.
• Maintain case records and ensure data is accurately entered in automated systems.
