Job summary

The Texas Health and Human Services Commission (HHSC) Community Services division seeks a Community Care Worker I to determine eligibility for Texans needing in-home care, meals, and other social services. You will conduct home visits, interview individuals, document findings, and refer for appropriate services.

Requirements include a high school diploma or equivalent and a valid driver’s license; willingness to travel up to 80% is essential.

Qualifications

• Knowledge of interviewing techniques to obtain personal information and resolve conflicting statements.
• Knowledge of resources that serve older or disabled individuals.
• Ability to effectively communicate orally and in writing.
• Ability to set priorities, establish timeframes, and meet deadlines.

Responsibilities

• Attends work on a regular and predictable schedule.
• Interviews applicants and individuals receiving services and conducts home/site visits to assess needs.
• Determines eligibility and functional needs for services.
• Obtains and verifies income/resources to determine financial eligibility.
• Maintains case records and enters information in automated systems.
• Monitors service effectiveness by contacting individuals and adjusting plans.
