Job summary

Qualifications

• Completed and detailed application illustrates writing ability.
• Willingness to travel at least 80% of the time.

Responsibilities

• Attend a regular and predictable schedule and perform other duties as assigned.
• Interview applicants and conduct home/site visits to assess service needs.
• Conduct reviews to determine eligibility and functional needs.
• Obtain and verify income and resources to determine financial eligibility for programs.
• Maintain case records and enter information in automated systems.
• Monitor to ensure services meet the individual’s needs.
• Develop and coordinate service plans with individuals, families, and providers.
• Prepare basic narratives or statistical reports.
