Job summary

Texas Health and Human Services Commission (HHSC) is seeking a Community Care Worker I to assess needs, determine eligibility, and authorize services for long-term care clients in Region 10 HHSC-CCSE Unit 17. The role handles a caseload, conducts home visits, and works with contract agencies to ensure services are delivered effectively.

The position requires high school graduation (GED) with travel up to 75%, basic computer skills, and strong communication.

Qualifications

• Application indicates any current customer service experience interacting with individuals or customers in a retail or social service setting.
• Application indicates willingness to travel a minimum of 75% of the time.
• Applicant has at least a High School diploma or GED; preference for 60+ college hours in Social Work, Psychology or Sociology.
• Experience using general office software such as Microsoft Office (Outlook, Word, Excel, PowerPoint) and SharePoint.
• Ability to communicate effectively in English; bilingual in English and Spanish preferred.
• Ability to work a regular first shift (Mon-Fri, 8:00am-5:00pm).

Responsibilities

• Interview clients or resources to assess needs and determine eligibility for Community Care Services.
• Manage a caseload and document information using automated systems.
• Coordinate and authorize services with contract agencies to meet client needs.
• Verify case data and explain program benefits and requirements to clients.
• Prepare basic narrative or statistical reports for management.
