• Determine eligibility for Community Care Services Eligibility (CCSE) program services.
• Conduct home visits and interviews to assess needs.
• Develop service plans and authorize services as appropriate.
• Maintain confidential records and input data in automated systems.
• Provide excellent customer service in person, by phone, and email.
