Qualifications

• Knowledge of interviewing techniques to obtain personal information and resolve conflicts.
• Knowledge of resources for older adults or individuals with disabilities.
• Ability to communicate effectively in writing and speaking.
• Ability to prioritize tasks, set timeframes, and meet deadlines.
• Ability to establish and maintain relationships with individuals, staff, and providers.

Responsibilities

• Attends regular and predictable schedule and performs other duties as assigned.
• Interviews applicants and conducts home/site visits to assess needs and provide referrals.
• Conducts eligibility reviews and determines financial eligibility for Title XX programs.
• Maintains and enters case records in automated systems.
• Monitors services to ensure they meet recipient needs.
• Develops and coordinates service plans with individuals, families, and providers.
• Prepares basic narratives or statistical reports.
