Job summary

Texas Health and Human Services Commission in Corpus Christi, TX is seeking a Community Care Worker I to determine eligibility for Texans needing in-home care, home-delivered meals and related social services. The role emphasizes compassion, policy understanding, and strong communication.

You will interview applicants, review documents, verify income, and coordinate service plans with clients, families and providers.

Qualifications

• Knowledge of interviewing techniques to obtain personal information, make inquiries, and resolve conflicting statements.
• Knowledge of resources that serve individuals who are older or have a disability.
• Ability to effectively communicate orally and in writing.
• Ability to set priorities, establish timeframes, and meet deadlines.
• Ability to establish and maintain effective relationships with individuals receiving services, coworkers, contract agency staff and staff from other federal or state agencies.
• Ability to operate computer and general office equipment.

Responsibilities

• Attend work on a regular and predictable schedule in accordance with agency leave policy and perform other duties as assigned.
• Interview applicants, individuals receiving services, and resource persons; conduct telephone and/or home/site visits to assess the need for services and provide information and referral to other resources.
• Conduct reviews to determine eligibility and functional needs.
• Obtain, verify, and calculate income and resources to determine financial eligibility for Title XX programs.
• Maintain case record documents and enter information in an automated system, establishing a record for each individual receiving service.
• Monitor through contacting individuals receiving services to determine if services are meeting the individual’s needs. Develop/coordinate/review service plans with individuals receiving services and their families, provider agencies and other state agency staff and authorize services appropriately and accurately to meet the individual’s needs.
• Prepare basic level ongoing or special narratives or statistical reports.
