Job summary

Qualifications

Responsibilities

• Attend regular and predictable schedule.
• Interview applicants and conduct home/site visits to assess needs.
• Determine eligibility and financial eligibility for programs.
• Maintain case records and enter information into automated systems.
• Monitor services to ensure they meet the individual's needs.
• Develop and review service plans with individuals and families.
• Prepare basic narratives or statistical reports.
