Job summary

Health & Human Services Commission (HHSC) Community Services division in Austin seeks a Community Care Worker I to determine eligibility for CCSE services. You will perform home visits, interview individuals, document information, and develop service plans to meet client needs.

Responsibilities

• Attends regular and predictable schedule as directed.
• Interviews applicants and conducts home/site visits to assess need for services.
• Determines eligibility for Title XX programs and documents findings.
• Maintains case records and enters data in automated systems.
• Monitors service effectiveness and adjusts plans as needed.
• Develops and coordinates service plans with individuals and families.
