Health & Human Services Commission (HHSC) seeks a Community Care Worker I to determine eligibility for Community Care Services and coordinate in-home care, meals, and emergency services for Texans. The role requires regular field visits, data entry, and collaboration with families, providers, and state agencies.

Responsibilities include interviewing applicants, verifying information, developing service plans, and monitoring outcomes to ensure needs are met.

Qualifications

• Completed and detailed application illustrates the applicant can communicate effectively in writing.
• Willingness to travel at least 65% of the time.
• Experience using email and other automated applications, such as Microsoft Office products.
• High school diploma or equivalent required.

Responsabilités

• Attends work on a regular and predictable schedule in accordance with agency leave policy and performs other duties as assigned.
• Interviews applicants and conducts home/site visits to assess needs and provides information and referral to resources.
• Conducts reviews to determine eligibility and functional needs.
• Obtains, verifies, and calculates income and resources to determine financial eligibility for Title XX programs.
• Maintains case records and enters information in automated systems.
• Monitors services to ensure they meet individuals' needs.
• Develops and reviews service plans with individuals, families, and providers; authorizes services appropriately.
