Job summary

Health & Human Services Comm in Texas is seeking a Community Care Worker I in Sweetwater to determine eligibility for in‑home care, meals, and social services. You will conduct home visits, interview clients, document findings, and develop service plans while coordinating with providers and state agencies.

Telework may be available, and travel up to 65% is expected. Eligible applicants must have a high school diploma or equivalent and be capable of using email and MS Office; strong communication

Qualifications

• High school diploma or equivalent required.
• Ability to travel at least 65% of the time.
• Experience using email and other automated applications, such as Microsoft Office products.
• Ability to communicate effectively in writing.

Responsibilities

• Determines needs and eligibility for CCSE services through home visits and interviews.
• Authorizes and monitors CCSE services to meet the individual’s needs.
• Maintains case records and enters data in automated systems.
• Provides information and referrals to other resources.
• Ensures authorizations are registered in automated systems.
