Job summary

Health at Home seeks a Community Care Coordinator to connect with physicians, discharge planners, senior living communities, and other healthcare professionals across the territory. You will drive outreach, education, and service coordination to ensure seamless care and strong relationships with referral sources and families.

You will develop marketing plans, support transitions, and maintain regulatory-compliant records while delivering patient-centered care and community education.

Qualifications

• 1 year sales/marketing experience is required.
• Experience in health care marketing is preferred.
• Desire to work with the elderly population.
• Valid driver’s license with auto insurance.

Responsibilities

• Develop referral relationships within the assigned territory to grow census.
• Build weekly, monthly, quarterly marketing plans and review with leadership.
• Support referrals from external discharge sources for smooth admissions.
• Explain services (Medicare home health, private duty, third-party coverage) to partners.
• Deliver community presentations to raise awareness of services.
• Evaluate clients post-referral to tailor plans of care and ensure orders.
• Communicate client information to clinical supervisors and coordinate supplies.
• Maintain records per regulatory standards and participate in growth activities.
