Health at Home is seeking a Community Care Coordinator in Port Saint Lucie, FL to serve as the liaison between the agency and local physicians, discharge planners, and senior living communities. You will drive referral growth, educate partners, and coordinate transitions into Home Health services, ensuring compliant, patient-centered care across your territory.

The role emphasizes outreach, relationship-building, and effective communication with clients, families, and healthcare professionals.

Qualifications

• 1 year sales/marketing experience required; healthcare marketing preferred.
• Demonstrated passion to work with the elderly population.
• Valid driver's license for agency state of operation with appropriate auto insurance.
• Exceptional verbal and written communication skills; strong presentation skills.
• Proficiency in Word, Excel, PowerPoint and CRM programs.
• Continuing Education requirements; participate in mandatory educational programs.

Responsibilities

• Proactively develop new referral relationships within your territory to grow census and meet goals.
• Build and adapt weekly, monthly, and quarterly marketing plans; meet with Administrator regularly.
• Coordinate referrals from external discharge sources to ensure smooth handoff into care.
• Understand full range of services (Medicare home health, private duty, third-party coverage) and communicate offerings clearly.
• Deliver presentations to community partners and residents to raise awareness of services.
