Job summary

Eskenazi Health seeks a Community Weaver to serve as a welcoming hub for patients and families seeking information or resources. You will connect patients with community programs, help navigate between departments, and provide tailored services to meet basic needs.

The role partners with Social Workers, Community Health Workers, and care teams to expand access to housing, food, clothing, transportation and utilities, while documenting visits and outcomes in a timely manner.

Qualifications

• High school diploma or GED with 3 years of community related experience.
• Community Health Worker certification preferred.
• Valid Indiana driver's license and Eskenazi driving policy compliance.

Responsibilities

• Act as a bridge to connect patients with trusted community organizations.
• Connect residents to a medical home and navigate services.
• Collaborate with Social Workers and Community Health Workers to improve community knowledge.
• Support neighborhood leadership activities and participate in supervision.
• Perform home visits and document outcomes promptly.
• Maintain therapeutic relationships and link patients to community resources.
