The role supports navigation to services and care coordination during inpatient, outpatient, and home visits, interacting with families to connect to community programs and resources.

The position requires a Bachelor's degree in health care/public health/related field, a valid Ohio driver’s license with insurance, and an active BLS certification, plus 2 years of patient-facing

• Bachelor’s degree in health care, public health, or related clinical field preferred.
• Valid Ohio driver’s license and auto insurance required.
• Active BLS certification required.
• 2 years of healthcare experience in a patient-facing role.

Responsabilidades

• Engage members to participate in case management programs and explain goals and benefits.
• Coordinate referrals and connect families to community resources.
• Provide navigation support and address social determinants of health.
• Communicate with the healthcare team and maintain case files.
• Schedule appointments and assist members to resolve immediate needs.
• Maintain records including eligibility, services, progress toward goals.
• Participate in staff orientation and continuing education.
