Nationwide Children’s Hospital is seeking a Case Management Extender to help navigate community resources and coordinate care across inpatient, outpatient, and home settings. The role involves patient and family engagement, and requires a Bachelor’s degree in a health-related field, a valid Ohio driver’s license, and a current BLS certification.

The position is full-time, Monday–Friday, 8:00 AM–5:00 PM, with benefits eligible.

Qualificações

• Bachelor’s degree in health care, public health, or related clinical field.
• Valid Ohio driver’s license and proof of auto insurance.
• Active BLS certification is required.
• 2 years of experience in healthcare in a patient-facing role.
• Must pass motor vehicle background inspection and maintain insurance guidelines.

Responsabilidades

• Engages and motivates members to participate in case management programs and supports family-centered care.
• Collects information for tailored assessments and refers when criteria for referral are met.
• Provides navigation support to help families access services and address social determinants of health.
• Outreach to PCP/POC, specialists, and home care providers to facilitate referrals.
• Schedules appointments and assists members to resolve needs in real time.
• Maintains records and case files with personal and eligibility information and progress details.
• Participates in orientation and continuing education of staff and students.
