Job summary

Nationwide Children's Hospital is seeking a Case Management Extender to help families access community resources and coordinate care across inpatient, outpatient, and home settings.

You will collaborate with providers, schools, and community programs to connect patients with services, educate families, and monitor transitions in care. Strong communication and knowledge of Medicaid are essential for success.

Qualifications

• Bachelor's degree in health care, public health, or related field.
• Valid Ohio driver's license and proof of auto insurance.
• Active BLS certification required.
• Working knowledge of Medicaid and regulatory standards.
• 2 years of healthcare experience in a patient-facing role.

Responsibilities

• Engage and motivate members to participate in case management programs.
• Develop family-centered care plans and collect assessments for eligibility.
• Facilitate communication among the healthcare team and with families.
• Provide navigation support to move clients through the healthcare system.

The Case Management Extender collaborates to arrange for or connect patients to needed services and identifies, creates, and nurtures relationships with local agencies, schools, churches, and other programs. Case Management Extenders are patient and family facing with daily activities during inpatient stays, outpatient specialty clinic visits, emergency room visits, home, community visits, and via telephone.Job Description:Essential Functions:Engages and motivates members to participate in the various case management programs by clearly articulating goals, benefits, and interventions. Assists with the development of family-centered care.Collects information for tailored assessments regarding case management eligibility and refers onward when response triggers criteria for referral and need for licensed clinical intervention. Facilitates communication and collaboration amongst the healthcare team.Provides personalized navigation support to members to help them move through the healthcare system. Connects families to resources to address social determinants of health and accommodates the specific cultural and linguistic needs of all patients. Manages and monitors transitions between settings, caregivers, and providers, providing follow-up across the continuum of care.Performs outreach to PCP/POC’s, specialists, and home care providers to research and facilitate referral for services. Develops patients and family’s self-management skills through education and resource provision.Answers incoming telephone calls, schedules appointments, and assists members to resolve immediate needs in real time. Manages administrative functions to support program.Prepares and maintains records and case files, including documentation such as clients' personal and eligibility information, services provided, progress towards goals, and significant changes.Participates in orientation and continuing education of staff and students as appropriate.Education Requirement:Bachelor’s degree with background in health care, public health, or related clinical field, preferred.Licensure Requirement:Valid Ohio driver’s license and proof of auto insurance as required by hospital policy and position-specific requirements.
