Responsibilities

• Help patients and families navigate and access community services and other resources.
• Support and maintain population health programs and care coordination activities.
• Identify, create and nurture relationships with local agencies, schools, churches and other programs.
• Interact with patients and families daily across inpatient stays, outpatient visits, ER, home and community settings, including telephonically.
