Qualifications

• 3-5 years of relevant customer service, client care, program coordination, administrative support, or community services experience.
• Strong communication skills and compassionate, respectful interaction with diverse clients.
• Excellent organization, follow-through, and attention to detail in records and documentation.
• Proficiency with basic computer systems, email, and data entry.

Responsibilities

• Assist participants with eligibility, enrollment, intake assessments, and ongoing support needs.
• Coordinate communication between clients, program staff, agencies, vendors, and partners.
• Maintain accurate client records, reports, and program documentation.
• Handle referrals, outreach, and scheduling of in-home assessments as needed.
• Support outreach efforts and connect clients to meal services or supportive programs.
