Pathway House in San Jose, CA is seeking a full-time Care Coordinator to serve as a liaison among clients, families, medical and mental health professionals, and criminal justice partners. You will support daily living activities, safety checks, and treatment engagement, ensuring a positive program experience.

Responsibilities include intakes, rounds, medication monitoring, and transportation coordination. Must be able to work in a fast-paced setting, pass background checks, and have a valid

Qualifications

• One-year practical experience in drug/alcohol or mental health field preferred.
• Excellent written and verbal communication skills and ability to work in a team or alone.
• Maintain highest professional ethics as described in employee handbook.
• Provide client services orientation with a professional demeanor and problem-solving attitude.
• Comfort with frequent interruptions in a fast-paced, client-centered environment.
• Able to physically respond to client needs, climb stairs, and move through the facility.
• Pass comprehensive background and reference checks; valid driver’s license preferred.

Responsibilities

• Assist with intakes, safety checks, and COVID testing.
• Conduct daily rounds, dorm inspections, and ensure client safety and participation.
• Assist with medication monitoring, documentation in system, and urinalysis.
• Educate clients on rules and procedures and refer to counselors as needed.
• Document client movement, arrange passes, and provide transportation to appointments.
• accompany clients to off-site appointments as needed and maintain continuity among teams.
• Perform additional duties as assigned by program director.
