Job summary

ADP, Inc. is seeking a Non-Clinical Care Navigator to enhance cancer care at the community level. The role focuses on helping patients navigate the health system, understand diagnoses, coordinate treatment plans, and access social services and care teams.

Under supervision of the Manager of Patient Intake, the Navigator screens for barriers, triages questions, and supports enrollment in programs while collaborating with providers and the care team. Strong communication and empathy are essential.

Qualifications

• Three years of experience directly related to the duties and responsibilities specified.
• Basic understanding of medical terminology and healthcare systems.
• Excellent written and verbal communication with patients and families.
• Knowledge of common office software and EMR systems.
• Detail-oriented and patient-focused.

Responsibilities

• Act as main point of contact for patients and families for support and questions.
• Perform follow-up calls after patient visits to review plans and reschedule as needed.
• Screen for barriers to access, distress, and comprehension of diagnosis.
• Assist with referrals to community resources and care coordination.
• Maintain professional relationships with multidisciplinary care team.
• Enroll and maintain patients in management and/or value-based programs.
