Job summary

Adagio Health seeks a Care Navigator to connect patients, clients, and communities with needed care, addressing social determinants of health and guiding enrollment in insurance and nutrition programs.

You will serve as a bridge between clinical services and community support, delivering practical health education and coordinating care with multiple teams. Emphasis on warm handoffs and detailed documentation.

Qualifications

• Accredited degree in Social Work/Public Health or 12+ months experience in community support or health insurance navigation.
• Active CHW certification or ability to obtain.
• Excellent verbal and written communication across diverse populations and clinical teams.
• Proficient with Microsoft Office and database systems.

Responsibilities

• Connect and guide clients through health, wellness and social determinants of health.
• Provide warm handoffs between clinical services, nutrition support and community services.
• Help community members enroll in health insurance as a Pennie Assister; support WIC enrollment and intake.
• Build relationships with local service orgs to increase awareness and access to services.
• Deliver practical health education and vetted resources for informed decisions.
• Collaborate across Clinical Services, Disease Prevention, and Nutrition to coordinate care.
• Maintain accurate, timely documentation of interactions, referrals and outcomes.
