Job summary

ADP, Inc. is seeking a Hospice & Palliative Care Navigator to lead goals-of-care and advance care planning for at-risk patients. The role emphasizes independent clinical judgment, collaboration with PCPs, and proactive education for families about hospice and palliative services.

The navigator will manage a high-risk patient panel, prioritize care, and ensure accurate documentation in the EHR, while maintaining knowledge of Medicare hospice guidelines and CMS requirements.

Qualifications

• Active RN or LPN license required in Ohio; BSN preferred, or 10+ years of LPN hospice experience.
• Minimum 3 years of clinical nursing experience; hospice/palliative care strongly preferred.
• CHPN credential preferred; experience in value-based care or care management is a plus.

Responsibilities

• Maintain a living registry of high-risk patients to determine hospice eligibility or palliative care needs.
• Independently prioritize and manage patient panel based on acuity and urgency.
• Build relationships with PCPs and provide clinically substantive recommendations.
• Lead goals-of-care conversations with patients and families in clinic or field settings.
