Job summary

Toledo Clinic is seeking a Non-Clinical Care Navigator (NCCN) to enhance cancer care at the community level. The NCCN will help patients anticipate, identify, and overcome barriers within the health care system, understand their diagnosis, coordinate treatments, and access social services while communicating with their treatment team and family.

Under the supervision of the Manager of Patient Intake, the NCCN will serve as the main point of contact for patients and families, perform follow-ups,

Qualifications

• Three years of relevant healthcare navigation experience.
• Strong written and verbal communication skills.
• Basic medical terminology and healthcare knowledge.
• Ability to use office software and EMR systems.

Responsibilities

• Serve as main point of contact for patients and families.
• Coordinate treatment plans and referrals to social services.
• Follow up with patients after visits to discuss plans and schedule appointments.
• Advocate for patients with the multidisciplinary team.
• Cross-train to handle scheduling, call center, and registration needs.
