Job summary

Claremedica is seeking an Access Community Benefits Coordinator to help patients access essential social services and navigate Medicaid, SNAP, and related programs. You will work closely with center admins and outreach teams to ensure seamless access to benefits and high patient satisfaction.

The role emphasizes program navigation, documentation in EHR, and collaboration across departments to provide holistic support and improve health outcomes for seniors in our communities.

Qualifications

• Experience with DCF Medicaid, SNAP, and other community programs.
• Strong customer service skills and ability to communicate effectively with clients.
• Multilingual abilities in English, Spanish, and Creole are a plus.
• Proficiency with Microsoft Office suite (Word, Excel, Outlook) and data entry.
• Excellent attention to detail and ability to handle multiple tasks.

Responsibilities

• Assist patients with Medicaid recertifications and redeterminations.
• Screen Medicaid eligibility and Dual Eligibility (Medicare & Medicaid).
• Identify and support Dual Eligible members to grow enrollment.
• Conduct New Member Orientations about available benefits and services.
• Generate leads for Access and refer to sales/enrollment teams.
• Maintain high Access engagement rate across centers and track outcomes.
• Assist with applications for SNAP, LIHEAP, Lifeline, LTC Waiver, STS, etc.
• Document all interactions and outcomes in internal systems and EHR with accuracy.
• Collaborate with clinical teams and community outreach staff to coordinate wraparound services.
