Pathways to Housing DC is seeking a Service Coordinator in Silver Spring, MD to join the Assertive Community Treatment team. This role involves outreach, case management, counseling, advocacy, and independent work in diverse environments.

The position emphasizes client-centered recovery, collaboration with a multidisciplinary team, and evidence-based practices to promote housing stability and well-being.

Qualifications

• Bachelor's degree in social work or related field or 2 years’ experience in mental health/human services.
• Patience, creativity, flexibility, compassion, and sensitivity to persons with disabilities.
• Driver's license and vehicle required; excellent communication, writing, and computer skills.

Responsibilities

• Collaborate with client in the formulation of an IRP reviewed with the client regularly.
• Provide outreach, case management, counseling, advocacy in various environments.
• Provide tailored housing, school, training & work opportunities, daily living support.
• Maintain written and computerized records and complete program documentation.
• Organize and participate in social activities with clients in the community.
• Coordinate referrals to community services and advocate client participation.
• Lead groups, workshops, and in-services.
• Meet MD Medicaid guidelines for assigned clients.
• Work effectively as part of a team.
• Utilize evidence-based practices.
