Job summary

Pathways to Housing DC is seeking a Service Coordinator in Silver Spring, MD to join the Assertive Community Treatment (ACT) team. You will collaborate with a multidisciplinary group to deliver housing-focused services, help clients secure housing, and provide recovery-oriented supports in diverse settings.

The role emphasizes compassion, flexibility, and independence in a fast-paced environment, with a focus on empowerment and consumer-driven care for individuals with complex needs.

Qualifications

  • Bachelor's degree in social work or 2 years’ experience in mental health services/human services.
  • Patience, creativity, flexibility, compassion, and sensitivity to persons with disabilities and other minority populations.
  • Career experience in social services, supported housing, or with persons with disabilities.
  • Excellent communication, writing, and computer skills.
  • Driver's license and vehicle required.

Responsibilities

  • Collaborate with client in the formulation of an Individual Recovery Plan (IRP) that is reviewed and modified with client on a regular basis.
  • Provide outreach, case management, counseling, advocacy, and other needed services to clients in any environment including: the streets, shelters, jails, hospitals, apartments, office, etc.
  • Provide individually tailored services to each client in the following areas: housing, school, training & work opportunities, activities of daily living, health, wellness self-management, relapse prevention, money management & entitlements, medication support, self-help & empowerment, problem solving, family life & social relationships and integrated treatment for substance abuse.
  • Maintain written and computerized records compile reports and complete other program documentation (e;g;, progress notes, incident reports, on-call logs, letters, psychosocial assessments).
  • Organize and participate in social activities with clients in the community;
  • Coordinate and monitor referrals to community services and advocate client participation;
  • Lead groups, workshops, and in-services
  • Meet assigned clients according to MD Medicaid guidelines
  • Work effectively and constructively as part of a team;
  • Utilize evidence-based practices;
  • Perform related work as assigned;

Pathways to Housing DC is an innovative agency based in the heart of Washington, D.C with services in Montgomery County Maryland. We believe that housing is a basic human right, and that without housing people are unable to address their mental health disorders, addictions, and other life stressors. We provide each person entering our program their own apartment, without requiring treatment compliance or sobriety as a prerequisite. We then provide each person with intensive support services to help them stay housed and begin their journey to recovery.

The Assertive Community Treatment (ACT) team provides flexible, community-based services that are designed to promote the stability, recovery and community reintegration of formerly homeless individuals who have a mental illness and co-occurring substance use disorders.

The Service Coordinator works in collaboration with the trans-disciplinary team (including Psychiatrist, nurses, addictions counselors, and peers) and provides ACT services for all clients on the team in a compassionate and responsible manner. All services consumers are consumer-driven and recovery-focused.

Successful Service Coordinators are flexible and patient. They work well in a fast-paced environment, and are independent thinkers.
