Job summary

Community Access seeks a Service Coordinator to provide engagement and comprehensive support to participants in its housing programs. You will work with individuals to support recovery, treatment access, social connections, and educational or vocational goals.

The role emphasizes group activities, documentation accuracy, and collaboration with team members to maintain stable housing for New Yorkers living with mental health concerns.

Qualifications

• Must have understanding and commitment to Community Access mission.
• High school diploma or GED required; bachelor’s degree preferred; degree may be substituted with relevant work experience.
• Strong written and verbal communication; ability to prioritize and meet deadlines.
• Ability to work independently and in a team; demonstrate initiative and confidentiality.

Responsibilities

• Provide engagement and comprehensive support services to participants in supported housing programs.
• Utilize person-centered approaches to help participants achieve recovery, treatment, education, or vocational goals.
• Lead individual support and plan community-building group activities.
• Complete documentation, service plans, and notes in a timely manner.
• Help ensure program compliance and data integrity across caseloads.
