Pathways to Housing DC is seeking a Mental Health Therapist for Montgomery County ACT Team in Silver Spring, MD. You will deliver clinical assessments, individual and group therapy in community and home settings, and coordinate admissions.

Requirements include Maryland licensure (LMSW/LGPC/LCSW-C/LCPC) and experience with psychiatric disabilities and trauma. A $1,000 recruitment bonus is available, with comprehensive benefits and housing-focused supports.

Qualifications

• Maryland licensed clinicians (LMSW/LGPC/LCSW-C/LCPC) with relevant certifications.
• Experience in case management or counseling of people with psychiatric disabilities, histories of substance abuse and/or trauma.
• Strong clinical skills in mental health and substance abuse.
• Patient, creative, flexible, compassionate and passionate about working with persons with complex needs.
• Experience in social services, supported housing or with persons with disabilities.
• Excellent organizational, communication, writing and computer skills.
• Bilingual English/Spanish preferred.

Responsabilités

• Complete clinical assessments on new and ongoing clients.
• Provide therapeutic services to consumers in the community and in their home.
• Provide community-based outreach services to homeless consumers.
• Provide individual, family and group therapy to targeted clients within the program.
• Maintain clinical documentation of all consumer visits in a timely and high-quality manner.
• Participate in the coordination of the consumer admission & assessment processes.
• Develop, implement and revise consumer treatment, crisis and discharge plans.
• Assist in teaching daily living skills and in housing stability and employment exploration.
• Complete monthly outcome reports on case plans.
• Provide other related duties as required.
