Qualifications

• Registered Nurse with ability to independently serve people with complex medical, behavioral and social needs.
• Experience in care coordination, case management, or dual-eligible populations preferred.
• Medicaid/Medicare managed care experience preferred.

Responsibilities

• Engage with enrollees in homes and community settings to build complex care management relationships.
• Lead interdisciplinary care teams to develop holistic care plans addressing medical and non-medical needs.
• Coordinate with primary care and community resources to ensure seamless care transitions.
