Job summary

Wisconsin Community Services is seeking a Case Manager for the Community Support Program (CSP) in Milwaukee. You will work with adults living with serious mental illness and co-occurring disorders, providing assessments, treatment planning, and connection to community resources.

Starting pay is $44,000 with opportunities for licensure supervision and ongoing clinical supervision within an ACT team. This full-time role emphasizes home visits and community-based service delivery.

Qualifications

• Bachelor’s or Master’s degree in behavioral science or related field.
• Knowledge of mental health and/or substance use disorders.
• Understanding of the DSM-5 and psychotropic medications.
• Valid driver’s license and reliable personal vehicle.

Responsibilities

• Complete ongoing assessments and develop person-centered treatment plans.
• Provide case management, supportive psychotherapy, symptom management, and crisis intervention.
• Monitor mental health symptoms and coordinate medication-related needs.
• Support participants in developing and achieving recovery goals.
• Coordinate mental health, healthcare, and substance use treatment services.
• Help participants access and maintain housing.
• Support money management and activities of daily living.
• Help participants pursue employment and educational goals.
• Assist with appointments, transportation, scheduling, and resources.
• Conduct frequent home visits and community-based outreach.
• Provide transportation when needed and drive agency vehicle when required.
• Work closely with participants’ families and natural supports.
• Participate on an Assertive Community Treatment (ACT) team.
