Job summary

North Central Health Care (NCHC) is seeking a Case Manager to provide community-based case management and rehabilitative support for adults and youth enrolled in CCS, CSP, CLTS, or CST. The role emphasizes recovery, independence, safety, and community integration through assessments, service planning, and coordinated care.

The position is full-time, in person in Langlade County, with 8:00am-4:30pm weekday hours, offering benefits including PTO, health and life insurance, and Wisconsin Retirement

Qualifications

• Bachelor’s degree in Human Services, Social Work, or a closely related field.
• More than 1 year of experience in case management.
• Must possess a valid Wisconsin driver's license and maintain unrestricted driving privileges throughout employment.
• Use of a personal vehicle for work purposes, mileage reimbursed for work-related driving.
• Must be authorized to operate any vehicle to transport clients, patients, or staff as required by the position.
• Required to obtain and maintain auto insurance coverage at the amounts required by North Central Health Care.

Responsibilities

• Conduct initial and ongoing assessments to identify mental health, substance use, functional, psychosocial, educational, and environmental needs.
• Coordinate services across multiple systems such as psychiatry, primary care, schools, courts, housing, AODA providers, crisis, and contracted services.
• Advocate for client needs to obtain medical, dental, legal, housing, financial, and community resources.
• Monitor progress and adjust interventions as needed to address symptoms, functioning, safety, or emerging needs.
• Provide side-by-side skill teaching in daily living areas such as cleaning, cooking, money management, hygiene, transportation, and community navigation.
• Use motivational interviewing and rehabilitative strategies to enhance coping, problem solving, social skills, symptom management, and independent living abilities.
• Assist clients in developing the skills necessary to seek, obtain, and maintain employment, including hygiene, punctuality, organization, job‑related supports, and transportation planning.
