Job summary

Wisconsin Community Services, Inc. in Milwaukee seeks a Case Manager for the Community Support Program (CSP). You will work with adults living with serious mental illness and co-occurring disorders, providing person-centered case management and supportive psychotherapy within your scope of practice.

This role offers hands-on supervision toward licensure, daily collaboration with an Assertive Community Treatment (ACT) team, and rich community engagement through home visits and outreach.

Qualifications

• Education: Bachelor’s or Master’s degree in behavioral science or a related field such as social work, psychology, counseling, or similar.
• Knowledge of mental health and/or substance use disorders.
• Understanding of DSM-5 and psychotropic medications.
• Strong interpersonal and communication skills.
• Ability to respond appropriately during crises and use de-escalation techniques.
• Knowledge of motivational interviewing and trauma-informed care is preferred.
• Organization, time management, and documentation skills.
• Professional boundaries and sound judgment.
• Ability to work with a diverse population.

Responsibilities

• Completing ongoing assessments and developing person-centered treatment plans.
• Providing case management, supportive psychotherapy, symptom management, and crisis intervention within your scope of practice.
• Monitoring mental health symptoms and coordinating medication-related needs.
• Supporting participants in developing and achieving recovery goals.
• Coordinating mental health, healthcare, and substance use treatment services.
• Helping participants access and maintain housing.
• Supporting money management and activities of daily living.
• Helping participants pursue employment and educational goals.
• Assisting with appointments, transportation, scheduling, and community resources.
• Conducting frequent home visits and community-based outreach.
• Providing transportation to participants when needed, including driving an agency van when required.
• Working closely with participants’ families and natural supports.
• Participating as an active member of an ACT team.
• Participating in group facilitation as assigned.
• Attending daily staffing, weekly team meetings, and in-service training.
• Maintaining accurate and timely clinical documentation per policy.
