Job summary

Assets, Inc. is seeking a dedicated Employment Case Manager (full-time) to lead service coordination, job development, and on-the-job coaching for individuals with developmental disabilities and/or mental illness in community-based settings.

The ECM collaborates with teams, develops ISP plans, maintains compliance, and supports employers while ensuring accurate documentation and client-centered outcomes. This role may involve flexible hours to meet client needs.

Qualifications

• High School Diploma or GED required.
• Associate degree (A.A.) or 2 years college coursework in human services or related field; and
• Two (2) years experience providing direct services and case management to people with developmental disabilities and/or mental illnesses.

Responsibilities

• Provide service coordination for persons supported by the team.
• Develop and implement Individual Support Plans (ISP) and coordinate treatment planning.
• Advocate for individuals served and maintain professional relationships with employers and staff.
• Coordinate off-site activities needed for job retention and transportation.
• Train, mentor and support staff to deliver quality employment services.
• Complete required documents and reports in a timely manner.
