Job summary

Assets, Inc. of Anchorage, Alaska is seeking an Employment Case Manager (ECM) to provide service coordination, job development, and direct support to individuals with developmental disabilities and/or mental health conditions.

This role collaborates with individuals, employers, and teams to ensure successful placement and ongoing service delivery. The ECM will coordinate services, develop job opportunities, provide training and coaching, and monitor ISP progress.

Qualifications

• High school diploma or GED required.
• At least one year of college coursework or equivalent experience.
• At least one year of experience in direct services, case management, or related field.

Responsibilities

• Coordinate services and support individuals in achieving employment goals.
• Develop job opportunities through community outreach, networking, and employer engagement.
• Provide job placement, training, and on-the-job coaching.
• Advocate for individuals and support long-term employment success.
• Develop, implement, and monitor Individual Support Plans (ISP).
• Track progress and adjust services based on outcomes and data.
• Conduct regular site visits to evaluate performance and support needs.
• Train, coach, and mentor Job Coaches and support staff.
• Maintain accurate and timely documentation (service notes, reports, plans, etc.).
• Lead or participate in team meetings and treatment planning.
• Use de-escalation and crisis intervention techniques when needed.
• Build and maintain professional relationships with employers, families, and service providers.
• Work a flexible schedule to meet client and program needs.
• Perform other duties as assigned.
