Qualifications

Bachelor's Degree in Arts/Sciences (BA/BS) in a related field; entry level.
Strong communication, problem solving and critical thinking skills; data entry and CP software knowledge required.
Preferred 1-3 years of relevant experience in care coordination, aging network, waiver programs, or managed care.
Ability to build relationships with diverse populations.

Responsibilities

Engage and assess elderly, frail clients in community settings; develop care plans and connect to resources.
Conduct assessments for program participants; provide case management and develop effective care plans.
Maintain patient files, document services, and ensure confidentiality; travel within assigned communities as needed.
Collaborate with internal stakeholders and participate in trainings; maintain mandatory certifications.
