Qualifications

• IDOA Certification or ability to obtain upon hire.

Responsibilities

• Engage and assess the complex needs of elderly and frail clients; develop care plans and connect to resources.
• Conduct timely assessments; provide case management and develop effective care plans.
• Maintain up-to-date client files and documentation; ensure confidentiality.
• Collaborate with internal teams; travel within assigned communities.
• Engage in ongoing learning, training and skill development; complete required certifications.
• Essential duties include other tasks as assigned.
