Job summary

Kahak Health Services is seeking a Community-Based Supports DSP to assist children with developmental disabilities in becoming more independent and integrated into their community. You will support participation in community activities and help practice skills learned in other settings, while ensuring safety and adherence to each child’s plan of service.

Role involves transportation to activities, daily living support, and documenting progress notes.

Qualifications

• CPR and First Aid certifications are required.
• Experience with children with developmental disabilities preferred.
• Resume and clear background check are required.

Responsibilities

• Support the child's participation in community activities and events identified on their plan of service
• Reinforce skills such as communication, socialization, personal care, mobility, relationship-building, and participation in leisure and community activities
• Assist with activities of daily living and support skill development in natural community settings
• Provide transportation to approved activities
• Monitor for safety and maintain a healthy, safe environment for the individual and yourself
• Follow each child's plan of service and complete required program documentation/progress notes for every visit
