Job summary

The role emphasizes assessment, goal setting, crisis response, and collaboration with the treatment team. A bachelor’s degree in a human services field and a valid driver’s license are required, with comprehensive benefits offered.

Qualifications

• Bachelor's degree in a human services field or equivalent work experience.
• Strong oral and written communication in English.
• Background check and drug screening as required.

Responsibilities

• Provide services in community settings where the consumer needs support.
• Provide outreach, referrals and connection to resources.
• Conduct strengths assessments with consumer and family.
• Develop and document treatment goals and plans; monitor progress.
• Advocate for client rights and maintain confidentiality.
• Coordinate day activities and crisis management as needed.
• Deliver individual therapeutic interventions and document outcomes.
• Enter and maintain clinical schedule in the EHR.
• Attend supervision and participate in program staffing.
• Complete required state training and work with the team.
