Qualifications

• Bachelor’s degree in a human service field or equivalent; one year substituting for education permitted.
• Strong oral and written communication skills in English.
• A valid driver’s license and automobile liability insurance as per policy.
• Basic personal computer skills enabling data entry and retrieval.

Responsibilities

• Provide services in community settings where the consumer may need support (school, work, home, etc.).
• Provide outreach, referrals to services, and connection of resources to consumer/family.
• Elicit and document strengths and goals with input from client and family.
• Develop and monitor client-centered treatment goals and plans with the treatment team.
