Qualifications

• Bachelor's degree in Social Work or related field.
• Licenses required: Licensed Professional Counselor and/or Licensed Social Worker.
• Demonstrates community engagement and problem-solving in a team setting.

Responsibilities

• Provide direct service in the WISE Program for Wayne & Holmes Counties.
• Coordinate with therapists to support youth and adults and reinforce treatment interventions.
• Monitor symptoms and stressors; provide crisis intervention as needed.
• Document billing in AVATAR and contribute to treatment planning and discharge summaries.
• Facilitate Wraparound meetings and provide in-home support to families.
