Job summary

Catholic Charities Diocese of Cleveland is seeking a Case Manager in Lake County to provide case management and supportive services to youth and families. Qualifications

• Bachelor's degree and 1–2 years related professional experience.
• Bilingual in Spanish preferred.
• Must have competent oral, written and interpersonal communication skills.
• Must be well organized and detail oriented; able to maintain confidentiality.
• Ability to work with diverse client population and staff at all levels.
• Fingerprinted final applicant per background check requirements.

Responsibilities

• Coordinate and deliver services consistent with Catholic Charities principles.
• Work with children, caregivers and parents to address unmet needs.
• Provide direct service in office, via telehealth or in the field/homes.
• Initiate and monitor interventions; reassess needs for successful outcomes.
• Develop community supports and partner with local agencies and resources.
• Advocate for clients and document records accurately in EMR and reports.
• Attend meetings and review treatment plans with supervisor.
• Perform additional duties to meet program objectives.
