Job summary

Home Base, Inc. is hiring a Case Manager/Wraparound Facilitator to support families in our Behavioral Health Program. The role involves coordinating wraparound services, collaborating with youth and families, and guiding a planning team through development and implementation of individualized plans.

The ideal candidate will have a human services background, experience in case management, and strong communication skills. Willingness to work evenings/weekends and drive to client sites is required.

Qualifications

• Bachelor’s degree in a human service field required.
• 2–4 years of experience in Case Management and/or outpatient/inpatient behavioral health setting preferred.
• 1–2 years of experience in leading teams preferred.
• Must communicate effectively and demonstrate experience.
• Computer skills including Outlook, Word, Excel, and PDF are essential.
• Must have a valid driver’s license and pass background checks.

Responsibilities

• Provide wraparound services to families with up to 15 members/families.
• Coordinate supports and resources for each member and family.
• Facilitate team meetings with youth, family, and team members.
• Ensure services and treatment modalities augment each other for optimal outcomes.
• Identify natural supports and community resources to meet family needs.
• Lead planning team through engagement, plan development, and implementation.
