Job summary

COMHAR, a nonprofit health and human service organization serving the Philadelphia region, seeks a Recovery Counselor/Case Manager to provide person-centered support and empower clients in their recovery journey. You will coach, mentor, and connect individuals to community resources to promote independence and well-being.

The role emphasizes warm, client-focused service delivery in a busy treatment setting with collaboration across a multidisciplinary team.

Qualifications

• B.A. in human service field with 2 years of mental health direct care experience or 60 hours of psychiatric rehabilitation training.
• Alternative educational paths accepted with relevant mental health experience.
• CPS certification qualifies for the role.

Responsibilities

• Support individuals in directing their own recovery process by promoting autonomy, choice, and self-advocacy
• Teach, coach, and motivate individuals in developing and applying skills that support recovery and daily living
• Educate individuals on available service options, including literacy programs, GED preparation, employment opportunities, volunteer work, and social activities
• Accompany individuals to access community services and resources as needed
• Escort individuals to essential appointments and services, including medical visits, hospitals, housing or shelter services
• Provide warmline coverage on a rotating schedule to offer support and guidance
• Coordinate transportation and support within the Welcome Center to access needed services
