As aCase Manager with our Community Support Program(CSP)and help adults living with serious and persistent mentalillness and co-occurring substance use disorders build stability, independence,and meaningful lives in their community.

Starting Pay: $44,000

What You’ll Do
As a CSP Case Manager, you’ll help participants move fromwhere they are today toward where they want to be.
Your responsibilities will include:
- Completingongoing assessments and developing person-centered treatment plans.
- Providingcase management, supportive psychotherapy, symptom management, and crisisintervention within your scope of practice.
- Monitoringmental health symptoms and coordinating medication-related needs.
- Supportingparticipants in developing and achieving attainable recovery goals.
- Coordinatingmental health, healthcare, and substance use treatment services.
- Helpingparticipants access and maintain housing.
- Supportingmoney management and other activities of daily living.
- Helpingparticipants pursue employment and educational goals.
- Assistingwith appointments, transportation, scheduling, and community resources.
- Conductingfrequent home visits and community-based outreach.
- Providingtransportation to participants when needed, including driving an agencyvan in circumstances required by program policy.
- Workingclosely with participants’ families and natural supports.
- Participatingas an active member of anAssertive Community Treatment (ACT) team.
- Participatingin group facilitation as assigned.
- Attendingdaily staffing, weekly team meetings, and in-service training.
- Maintainingaccurate and timely clinical documentation in accordance with agencypolicy and DHS 63.
- Managingparticipant funds when appropriate.
- Collaboratingwith teammates to address both planned and unexpected participant needs.

About70% of your work happens in the community andparticipants’ homes.
You may spend your day helping someone move into stablehousing, accompanying a participant to a medical appointment, connectingsomeone with substance use treatment, coaching someone through an importantlife skill, or helping de-escalate a crisis.
You’ll work in real-world environments where progress isn'talways linear.

What You’ll Bring
Education
- Bachelor’sor Master’s degree in behavioral science or a related field, such as:
- SocialWork
- Psychology
- EducationalPsychology
- Counseling

Skills & Knowledge
- Knowledgeof mental health and/or substance use disorders.
- Understandingof, or willingness to learn, the DSM-5 and psychotropic medications.
- Stronginterpersonal and communication skills.
- Abilityto respond appropriately during crises and utilize de-escalationtechniques.
- Knowledgeof motivational interviewing and trauma-informed care is preferred.
- Strongorganization, time management, and documentation skills.
- Abilityto manage multiple responsibilities in a fast-paced environment.
- Abilityto maintain professional boundaries and exercise sound judgment.
- Abilityto work effectively with a diverse population.

- Validdriver’s license.
- Reliablepersonal vehicle and sufficient insurance to meet agency requirements.
- Abilityto pass the required caregiver background check.
- Abilityto travel throughout Milwaukee County and work in participants’ homes andcommunity settings.
Schedule
First Shift | Generally 8:00 a.m.–4:00 p.m.
- Approximately40 hours per week.
- Flexibleschedule required based on participant and team needs.
- Someweekends and holidays.
- Rotatingafter-hours hotline coverage.
- Approximately70% community-based.
- Frequentparticipant transportation and home visits.
- Somedays may include up to approximately five hours of driving.
The position requires the ability to navigate stairs andperiodically lift/carry approximately 20 pounds to assist participants withvarious tasks. Reasonable accommodations may be made to enable individuals withdisabilities to perform essential functions.
